Advance Directive & Living Will Registry

Hemlock Society of San Diego has partnered with the U.S. Living Will Registry to provide its members free registration in this nation-wide Advance Directive Registry. Registration allows your hospitals and health care providers easy, instant access to your Advance Directive by secure internet or automated telephone/fax system. The Partnership helps our organization in its mission to increase public awareness about Advance Directives and to encourage their use.


There are two types of legal Advance Directives. First is the Living Will, in which you record the types of treatment you want or don't want. Second is the Health Care Proxy, in which you name a trusted person to make decisions for you if you are incapacitated.


"Our service is unique in that it provides lifetime membership, so there is no need to worry about renewing every year," says Dr. Joseph Barmakian, Founder and President of the Registry. "We send a yearly update letter to each registrant, allowing them to update their personal and emergency contact information and to confirm that their Advanced Directives still reflects their wishes."


Established in 1996, the U.S. Living Will Registry electronically stores Advanced Directives and Organ Donor information. This information is made available 24 hours per day to participating hospitals and health care providers. Hemlock Society of San Diego has registered as a community partner organization and because of this affiliation, the service to our members is free of charge.
